Medtronic Matching Gift Program and Volunteer Grant Information

Through Medtronic's Mission In Motion program, Medtronic matches donations made by all global employees and retirees to most nonprofits including:

• Higher education
• K-12 schools
• Arts & cultural organizations
• Environmental organizations
• Civic and community organizations
• Health and human services
• And most other 501(c)(3) organizations

Medtronic's Matching Gift Submission Process:

a. Online Giving
Employees can either make their original donation through the company platform or visit {{forms}} to submit a matching gift request after making the donation to the nonprofit.

b. Offline Giving
Each donation you make outside of the Volunteer and Giving site must be recorded individually within 90 days of the donation date to be eligible for Matched Giving. This includes recurring donations. The four acceptable forms of proof of donation are:

- An uploaded proof of donation. Proof of donation must include: the name of the donor (Medtronic employee), donation amount, donation date, and name of the eligible nonprofit organization on a receipt of payment from the organization
- Bank statement
- Confirmation email, proof of donation, or thank you letter from the nonprofit organization
- Canceled check (front and back of check endorsed by the nonprofit organization)

The proof of donation/receipt must be an original, unaltered document, and all information on your Matched Giving request must be the same as the information on the proof of donation/receipt. Pledges without proof of receipt, pooled donations, and in-kind donations (purchased items) are not eligible.

Medtronic offers a volunteer grant program to recognize employees, retirees and volunteers who give their time to nonprofits.

After individuals volunteer with an organization for at least 10 hours, Medtronic provides a volunteer grant of $200 to the organization. Medtronic will provide grants up to $1,000 for up to 50 hours.
